Ocular Surface Optimisation Before Premium IOL Implantation

The ocular surface, meaning the tear film and the outer layer of the cornea, is the first surface light passes through. If it is irregular from dry eye or eyelid inflammation, corneal measurements, lens power and astigmatism axis can change, and visual quality after surgery may suffer. It is therefore assessed before toric, EDOF and multifocal lenses, treated when needed, and measurements are repeated.

Quick answers At a glance

Why does dry eye matter before cataract surgery?

It can make corneal measurements inconsistent, affecting lens power and toric axis calculations, and it can reduce clarity and comfort after surgery.

Will treating dry eye delay my surgery?

Sometimes, for a short period, until the surface settles and measurements are repeated. The aim is a more accurate calculation, not unnecessary delay.

Does dry eye rule out a multifocal lens?

Not always. It is treated first and then reassessed, but severe or persistent dry eye may make a multifocal lens unsuitable.

What is the ocular surface?

The ocular surface includes the tear film, the corneal epithelium, the conjunctiva, and the eyelids with their oil glands, which produce the oily layer of the tears. The tear film is the first optical surface light meets before reaching the cornea and lens.

When it is smooth, vision is clear and measurements are stable. With dry eye or eyelid margin inflammation, the surface becomes irregular and vision can fluctuate from moment to moment.

How does dry eye affect lens measurements?

IOL power calculation depends on accurate corneal curvature measurements. Studies have shown that an unstable tear film is associated with greater variability in keratometry between readings, which can carry through to the chosen lens power.

  • Astigmatism may appear larger or smaller than it really is, or its axis may shift.
  • Readings may differ between devices or between visits.
  • Toric lenses are especially sensitive, because they depend on both the amount and axis of astigmatism.

When repeated measurements do not agree, treating the ocular surface and measuring again is an important step before the final lens is chosen.

Why does this matter more with premium lenses?

Multifocal and trifocal lenses divide light between distances and EDOF lenses extend the range of focus; both rely on good optical quality to work well. An irregular surface can reduce sharpness and contrast and increase glare or fluctuating vision.

Some patients attribute dissatisfaction after surgery to the lens when untreated dry eye is the real cause. This is why the surface is assessed before these lenses are recommended.

How is the ocular surface assessed?

Depending on your eyes, the assessment may include:

  • Questions about burning, gritty sensation and fluctuating vision.
  • Slit-lamp examination of the eyelids and oil glands.
  • Tear film stability and corneal surface staining where indicated.
  • Comparison of repeated corneal measurements and map regularity.

More detail is available on the dry eye treatment page.

How is the surface treated before surgery?

Treatment depends on the cause and severity and may include:

  • Lubricating drops, often preservative-free.
  • Warm compresses and eyelid hygiene for oil gland dysfunction.
  • Anti-inflammatory treatment where appropriate, under medical supervision.
  • Additional in-clinic dry eye treatments in selected cases.
  • Management of any other surface condition affecting corneal regularity.

Once the surface improves, measurements are repeated. If they agree, lens selection and surgery planning can proceed.

Realistic expectations

Important: dry eye can temporarily worsen after cataract surgery in some patients, so treatment sometimes continues afterwards. With severe or non-responsive dry eye, a monofocal or toric lens may be more suitable than a multifocal lens.

Lens selection follows an individual examination and discussion of your needs; a specific lens cannot be recommended online. See also the role of corneal tomography in planning.

Questions patients ask

Can I have dry eye without noticing it?

Yes. Some patients, especially older adults and people with diabetes, have clear signs of dry eye on examination with few symptoms, yet it still affects measurement stability.

How long does ocular surface treatment take before surgery?

It varies, but reassessment often takes place after a few weeks of regular treatment. Timing is decided once the eye has responded and measurements agree.

Should I stop eye drops before my measurement appointment?

You may be asked to avoid certain drops or ointments shortly before measurements, as they can affect readings. Follow the clinic's instructions for your appointment.

Will dry eye return after surgery?

It can worsen temporarily after surgery in some patients. It is usually manageable with drops and eyelid care and is reviewed during postoperative follow-up.

Can I have a toric lens if I have dry eye?

Often yes, once dry eye is treated and astigmatism measurements and axis are confirmed to be stable, because toric lens accuracy depends on them.
